About this website

This website provides general information about CareBridge’s RN clinical assessment, documentation, quality review and coordination support for New York Licensed Home Care Agencies. It also lets you request a meeting or apply to our nurse network.

The content on this website is for general information only. It is not medical, clinical, legal, or other professional advice, and it is not a substitute for professional judgment or care.

Nothing on this website creates a provider-patient relationship or any professional relationship.

Contacting us does not create a service agreement

Submitting a form or contacting us does not, by itself, create a contract for services.

Meeting scheduling is handled through Cal.com. Choose an available time and complete the booking steps; a meeting is booked only when the calendar confirms it.

Any services CareBridge RN provides are governed by a separate written agreement between CareBridge RN and the client. If anything in these Terms conflicts with that agreement, the separate agreement controls for those services.

Website forms and communications

Public website forms are for agency inquiries and nurse applications, not patient assessment requests or medical records. Do not submit patient names, assessment findings, or other confidential patient information through these forms. Agencies should use the approved secure email process arranged with CareBridge.

CareBridge uses SMS in its business communications. Providing a phone number through this website does not by itself grant blanket consent to marketing messages. For questions about messages or to request that SMS contact stop, email assess@carebridgern.com.

Nurse applications

Applying to the CareBridge Nurse Network does not guarantee any engagement or employment. Any engagement is subject to a separate agreement, credential verification, and applicable requirements.

Intellectual property

You may browse the website and download documents offered for personal or internal agency review. Preserve document notices and attribution. Sample clinical and legal materials are illustrations, not issued patient records, treatment orders or agreements. Other uses must respect the rights of the relevant copyright owner and any applicable license.

CareBridge branding and third-party names, logos and materials remain subject to their respective owners’ rights. Displaying a partner logo does not grant permission to reuse it or imply endorsement of every statement on this website.

Acceptable use

You agree to use this website only for lawful purposes. You agree not to:

• Disrupt, damage, or attempt to gain unauthorized access to the website or its systems.
• Submit false, misleading, or harmful information.
• Use the website in any way that violates applicable law or the rights of others.
