CareBridge RN ('CareBridge', 'we', 'us', or 'our') provides RN assessment, clinical documentation, and coordination support to licensed home care agencies across New York.

Information we collect

We only collect the information you choose to give us when you submit a form on this website. You do not need to create an account to use the site.

• Agency inquiries (Request Coverage or Contact forms): agency name, your name, your title, email address, phone number, your average monthly assessment volume, your primary challenge, and anything you write in the message field.
• Nurse applications (Join the Nurse Network): your name, email address, phone number, RN license status, home care experience, availability, preferred coverage area, and anything you write in the message field.

Automatic technical information

Like most websites, our hosting provider may automatically record limited technical information — such as your IP address, browser type, and the pages you visit — to keep the site secure and running reliably. We do not use this information to personally identify you.

How we use your information

We use the information you provide to:

• Respond to your inquiry and follow up about our services.
• Review and consider nurse applications.
• Communicate with you about a request you have made.
• Operate, maintain, and improve this website and our services.

How we store and share your information

When you submit a form on this website, your details are sent through our form-processing service (Web3Forms) and delivered to us by email. We keep that information in our email and business systems, with access limited to authorized CareBridge staff who need it to respond to you.

We do not sell your information, and we do not share it with third parties for their own marketing. We may share it with trusted service providers who help us process form submissions and operate our email and business systems (such as Web3Forms), and with others when required by law or to protect our legal rights.

Cookies and analytics

This website does not currently use cookies or third-party analytics or advertising trackers.

If we add website analytics (such as Google Analytics) in the future, we will update this Privacy Policy and, where appropriate, show a cookie notice so you can manage your preferences.

Health information and HIPAA

This website is for general information and business inquiries only. It does not collect patient health information (PHI).

In our work supporting home care agencies, CareBridge RN may handle protected health information as a 'Business Associate' under HIPAA. That information is governed by our Business Associate Agreements and privacy practices with those agencies — not by this website Privacy Policy.

How long we keep your information

We keep the information you submit only as long as we need it for the purposes described above and as required by law, after which we delete or de-identify it.

Your choices and rights

You can ask us to access, correct, or delete the information you submitted, or ask us to stop contacting you, at any time. Just email assess@carebridgern.com and we will respond as required by applicable law.

Security

We use reasonable administrative, technical, and physical safeguards to protect your information. However, no method of transmitting or storing data is completely secure, so we cannot guarantee absolute security.

Children's privacy

This website is intended for businesses and working professionals. It is not directed to children, and we do not knowingly collect information from anyone under 18.

We may update this Privacy Policy from time to time. When we do, we will revise the 'Last updated' date at the top of this page.
