Application, Review & Assignment Readiness

Onboarding starts with an invitation from the CareBridge office. Use the public application above to express interest; our team provides the access instructions for your profile and credential review.

01

Receive Your Invitation

The office adds your name, mobile number and email, then sends an invitation. Open the link on your phone and set your password. Invitations are designed to expire after seven days; the office can resend them.

02

Complete Your Profile

Add your address, service areas, languages and optional profile photo. Upload your RN license and supporting health records, check the extracted details, and select your usual hours and visit types.

03

Submit for Office Review

Review your information and send it to the office. If corrections are requested, update the marked items and resubmit. Your other information stays in place.

04

Approval & Visit Readiness

Office approval makes your profile active for assignment coordination. Confirm agency-specific credential requirements, orientation and visit terms before your first assignment.

Your App Profile

About you

Choose the boroughs and areas you cover, add languages and an optional photo, and enter your home address for assignment matching. The address is not shown to patients. Other areas and languages can be entered when needed.

Credentials & review

The initial profile requests your New York RN license, physical, TB and flu records. Supporting health records can be added later in the profile flow; missing items remain on your to-do list. Profile approval does not waive the credential requirements of a patient-care assignment.

Availability & updates

Set weekly working hours and visit types, then block time off in Calendar. Track invitation, waiting-for-approval, changes-requested and active status. In-app alerts and email support review; this flow does not use SMS or GPS tracking.

RN Credentialing & Continuing Compliance

Prepare the evidence needed for professional review, agency orientation and assignment clearance. The overview below distinguishes initial preparation from records that must remain current.

Checkmarks identify records for review where applicable to the engagement and receiving agency. Ongoing review follows applicable rules, expiry dates and agency policy; it does not imply a fixed monthly or annual schedule. Name-change evidence and declinations apply only where relevant. The app includes an optional profile photo, not live selfie verification. This checklist covers credential review; the initial app profile collects the RN license, physical, TB and flu records. Additional requirements are confirmed before assignment.

  • Your assignment

Confirm the visit type, patient location, agreed time and the assessment responsibilities.

  • Your documentation

Confirm the required forms and submission process. Our documentation turnaround target is 24 hours after the visit; the client agency’s DPS approves every Plan of Care before finalization.

  • Your support contacts

Save the coordination contact and confirm how to report access problems, cancellations or clinical concerns.

  • Your payment arrangements

Review the visit rate, payment timing and any applicable travel or cancellation terms before accepting.

Your Onboarding Questions

What should I have ready before applying?

Start with your contact details, RN license status, clinical experience, availability and preferred service areas. The initial application does not ask you to upload credential documents.

Which documents will I need?

Have your RN license details and photo identification available, along with relevant training, health and professional insurance records. The team will confirm the documents required for your role and the agency’s assignments before you send them.

How should I send my credentials?

Invited nurses upload photos or PDFs to their credential profile and check the extracted numbers and dates. Follow the access instructions provided by the onboarding team. Do not upload sensitive documents to the public application form.

Can I apply without home care assessment experience?

Share your clinical background and experience level. The team will discuss the work, orientation and whether available assignments are suitable for your experience.

How long does onboarding take?

Timing depends on the information required, credential review and assignment needs. The team will explain any outstanding steps; submitting an application does not confirm clearance for a visit.

When will I know the assignment rate?

Rates are disclosed after onboarding approval. Review compensation, payment timing and any applicable travel or cancellation terms before accepting an assignment.

What if my credentials or availability change?

Use your app profile to replace credential files, update your information and manage your usual working hours. Missing or expiring records appear for you and the office to review. Use Calendar to block specific days or part of a day. Profile changes after approval return to the office for review. Contact the team if you need help.
