A new concern. A focused clinical review.
A focused registered nurse (RN) assessment documents a reported change in the patient’s condition or function and provides findings to support agency follow-up.

Tell the coordinator what changed, when it was noticed and who reported it. A change in mobility, skin condition or day-to-day support may call for a focused visit between scheduled assessments. The referral should explain the clinical question, rather than assume its cause.
The RN documents and signs the assessment, including relevant history, observed findings and information that could not be established. Concerns follow the agreed escalation process. Our office reviews the documentation and coordinates any required corrections before delivery. This arranged service is not an emergency-response service.
Visit Focus and Agency Handoff
The agreed scope guides preparation, the RN’s visit record and follow-up with your agency.
Describe the concern
Describe when the change was noticed and how it differs from the patient’s previous condition, where known. Separate patient or caregiver reports from verified information and identify the contact responsible for clinical follow-up.
Assess the agreed scope
The RN examines the areas relevant to the referral, compares available previous information with current findings and records limitations and any action taken. The RN verifies the record before signing.
Support the next decision
Your agency receives a reviewed record to inform follow-up, further clinical advice or an authorized care-plan change.
The completed visit record is delivered as a PDF or by authorized entry into your agency’s electronic health record (EHR), as agreed. Where a Plan of Care is prepared or revised, your agency’s Director of Patient Services (DPS) approves it before finalization.
