Before services restart. Review the current needs.
Following a service interruption, a registered nurse (RN) assessment documents current care needs and changes from the previous plan to inform arrangements for resuming services.
Following a service interruption, a registered nurse (RN) assessment documents current care needs and changes from the previous plan to inform arrangements for resuming services.
A service interruption may follow hospitalization or another gap in care. Explain why services stopped, how long they were interrupted and what is known to have changed. Share the earlier care plan, recent clinical information and current orders through the document-sharing process agreed with your agency.
The RN records the patient’s present condition and differences from the earlier record, then verifies and signs the assessment. Our office reviews the documentation and coordinates any required corrections before delivery. Your agency and the appropriate practitioners or payers remain responsible for decisions and authorizations to restart services.
Visit Focus and Agency Handoff
The agreed scope guides preparation, the RN’s visit record and follow-up with your agency.
Describe the interruption
Provide the earlier service arrangement and relevant events during the gap, including hospital care or changes in caregiver support.
Review present needs
The RN compares current function, reported concerns and available support with the earlier care plan, within the agreed visit scope. Changes affecting the proposed restart and information requiring further confirmation are documented.
Inform the restart plan
Use the reviewed findings to consider changes to the care plan. Assessment delivery does not itself authorize services to resume.
The completed visit record is delivered as a PDF or by authorized entry into your agency’s electronic health record (EHR), as agreed. Where a Plan of Care is prepared or revised, your agency’s Director of Patient Services (DPS) approves it before finalization.
Care Resumption Documentation
These samples illustrate documents that may support a follow-up visit. The delivered record depends on the assessment type, applicable requirements and the scope agreed with your agency.
Illustrative samples with fictional details. Required forms vary by assignment; these are not issued patient records, treatment orders or legal agreements.
