Upload your records. Check the details.

The credential screen accepts document photos and PDFs, then helps read numbers and dates for you to verify. Missing items remain visible for follow-up. The office reviews applicable credentials before assignments.

A record skipped during profile setup is still required before assignment when it applies. See the full onboarding checklist below.

Arrive with the details you need.

The office handles patient outreach and scheduling. Your visit card brings together the confirmed time, assessment type, agency documents and practical access notes. On arrival, record the time with a tap. Contact the office if you cannot attend or the patient is not home.

CareBridge’s agreed flow uses an arrival timestamp, without GPS tracking. Sample screens contain no real patient details.

Start with the medication information.

The visit flow begins with photographs of medication bottles, lists or discharge papers. Capture the relevant labels, retake unclear images and continue to the review screen. Extracted information needs your verification.

08Vitals & body systems

Document findings as you assess.

The assessment screens support speaking, typing and photographs where appropriate. Review captured vitals and work through each body system. Confirm normal findings only when assessed, record symptoms and document the intervention or follow-up for identified concerns.

The workflow flags missing or unclear information for review. It does not replace clinical judgment or urgent escalation.

09Narrative & agency note

Keep your observations in your own control.

Capture your visit narrative, then review any suggested wording before keeping it. Record information the agency needs to follow up on in a separate note. That note accompanies the report for the office to review and communicate to the agency.

Check that suggested wording adds no unsupported findings. The nurse does not send the agency email from this screen.

11Office review & follow-up

Know what happens after you send.

The office checks the submitted report and follows up with you if a correction is needed. After review, the office handles delivery as a completed PDF or authorized entry into the agency’s EHR, as agreed. A correction request returns to Home and the delivery status updates after completion.

Submission, clinical review and agency delivery are separate stages.

Questions From Nurses

How are visits assigned?

Our office coordinates patient outreach and scheduling, then matches assignments to approved nurses using availability, service areas, languages and visit requirements. This is an office-coordinated workflow rather than an open list of visits to book yourself.

Is the nurse app available now?

Yes. CareBridge’s platform supports the assessment workflow, including documentation drafts for RN review, verification and signature. Start with the public application; our office provides onboarding instructions and access. The walkthrough shows illustrative screens, not a live clinical workspace.

What happens if my documents need updates?

The office will explain what needs correction or renewal. Your profile keeps missing and expiring records visible, lets you resubmit corrections and routes changes for review. Applicable credential requirements must be satisfied before assignments.

Can I apply if I already work full-time?

Yes. Tell us whether you are looking for part-time, per-diem, weekend or other availability. Our team will discuss opportunities that fit your schedule and preferred service area.

What kinds of visits would I complete?

Our assessment work includes initial visits, recertifications, post-hospitalization assessments, changes of status and supervisory visits. Your experience and the assignment requirements guide the match.

Do I need a New York RN license?

You must be authorized to practice as an RN in the state where the assignment takes place. For our New York work, the team verifies appropriate New York licensure before assigning visits. You can share your current license status in the application.

What if I am new to home care assessments?

Include your clinical background and experience level when you apply. We will discuss the assessment responsibilities, orientation and whether available assignments are a suitable fit.

Who handles the documentation?

You complete the clinical assessment and provide the findings. Our platform prepares documentation drafts for your review, verification and signature. Our documentation team assists with preparation and corrections, followed by clinical quality review. You retain responsibility for your clinical findings and signed documentation.

How are rates and payment arrangements confirmed?

Rates are standardized by visit type and disclosed after onboarding approval, before you accept an assignment. Confirm payment timing and any applicable travel or cancellation terms with our team.

Do I need to upload documents to apply?

No documents are requested in the initial application. Start with your contact details, license status, experience and preferences. Our team will explain the next steps for providing credentials.

Who can help if a visit does not go as planned?

Your coordination team can help with scheduling changes, access issues and agency communication. During onboarding, confirm your support contacts and the process for escalating clinical concerns.
