Focused assessment support

A specific concern.
A visit with a clear purpose.

For a specific patient concern, CareBridge coordinates a focused registered nurse (RN) assessment with an agreed clinical question, visit scope, documentation requirements and follow-up contact. Our coordination team confirms these requirements with your agency before arranging the visit.

Plan the visit around the referral

01

Describe what needs attention

Examples include a reported skin change, a fall, a change in daily function or concerns about support at home. Tell us when it started and who reported it.

  • Distinguish reported information from verified findings
  • Identify the questions your agency needs answered
  • Share patient information through the document-sharing process agreed with your agency
02

Agree the scope and timing

The coordinator reviews the request, available information and RN coverage with your agency. Confirm the patient’s availability and any access requirements before the appointment.

  • Relevant history and orders
  • Clinical contact for questions or escalation
  • Requested assessment date and documents
03

Record findings and limitations

The RN assesses the agreed areas and documents observations, relevant history and any information that could not be established. Urgent findings follow the agreed clinical escalation process.

  • Actual findings and actions taken
  • Patient or caregiver reports clearly identified
  • Any limits on completing the assessment
04

Receive the reviewed record

The RN verifies and signs the assessment. Our office reviews the documentation and coordinates any required corrections before delivery. Your agency’s clinical team decides the next steps and obtains any necessary orders or service authorizations.

  • RN-signed assessment and applicable supporting documentation
  • Follow-up questions or unresolved information
  • Delivery as a completed PDF or authorized entry into your agency’s electronic health record (EHR), as agreed
  • Where a Plan of Care is prepared or revised, your agency’s Director of Patient Services (DPS) approves it before finalization

Discuss a focused visit

Start with your agency name, service area and general visit requirements. Do not include patient details in a public inquiry. This is not an emergency-response service; call 911 for an immediate medical emergency.
